Multi-Drug Resistant TB

-One of the most fatal dz in the world- 1.86 billion died in 1997

MDR= strain of TB that is resistant to 2 of the 4 baseline drugs to Tx TB (Isoniazid, Rifampin, Ethambutol, pyrazinamide)

MDR is caused by: Poor Pt compliance
                    Long treatment program for TB up to 3 months and the pt starts to feel better by a few weeks
                    Poor public health planning/aid- country running out of funds to have a continuous supply of TB drugs

Tx of MDR: Expensive and complicated. Cost about 100,000 dollars to tx one pt with second line Antibiotic drugs that have severe side effects on the pt. Additional MDR has only a 50% cure rate.

World Health Organization (WHO) - predicts that by 2015 over 1.6 million people will be tx for MDR strain of TB
and by 2010 25 countries will be compliant to WHO's MDR-TB tx guidelines

- 85 and older group is fastest growing
- cataracts and ARMD common eye problems with older population
- review of cataracts/armd/diabetes+htn ret

increase in obesity in kids linked to Type II diabetes

In the past decade, there has been an increase in the number of kids
with type II diabetes (formerly known as Adult-onset diabetes
mellitus).  Several studies indicate that this stems from an increase
in obesity in this same population from inactivity, poor nutrition and
poor patient education



HP2010 diabetes goal: through prevention programs, reduce the disease
and economic burden of diabetes, and improve the quality of life for
those who have diabetes or at risk for developing diabetes.



Objectives: increase proportion of kids who participate in good
physical activity at school and home, decrease number of sedentary
hours (i.e. playing video games, watching tv, etc), increase proportion
of kids who eat good meals at school--- overall goal is to decrease the
number of kids who are obese.  This will hopefully reduce the number of
kids who develop type II diabetes and make the quality of life of those
who do have the disease a little better.



HP2010 Progress report in 2005: almost reached goal or have exceeded
goal in diabetes and physical activity category; still far from
achieving goal is nutrition category

 Schizophrenia and Public Health
1. Schizophrenia affects about 1% of the population. Theres both a genetic component and environmental component
2. Signs and Symptoms are grouped into positive and negative. Positive symptoms are acquired like delusions and hallucinations. Neg symptoms are loss of affect and mood.
3. Treatment involves dopamine antagonist medication. Pt will probably be on medication for the rest of their lives.
4. Prevention is very difficult because of low incidence and non specific triggers. Health promotion agains comorbid conditions is difficult because pt dont see the benifit if the hallucinations and delusions persist.

Walk with a pedometer cuz it's good fer ya, dawg.
Talk was about HP2010 Obj. 5-13, 28-1, 28-5.
To reduce DR from showing up, reduce blood pressure to normal (120/80)
Using a ped. results in walking one more mile per day.
less than 5000 steps = sedentary (me)
             5000 - 7499 = low active

             7500- 9999 = somewhat active (you)

             10,000 steps is the goal.

Go get 'em Chief Rays!!! Public Health is for winners!!! Thanks Steve for setting this up.

Topic: Tobacco Use in Teens (objective 27)

Key points:
   - lots of health consequences of smoking
   - 90% of smokers began smoking before age 21
   - there's been a decline in smoking rates from 1997 to 2005
   - most teens who smoke tried to quit but are unsuccessful
   - smoking cessation programs are effective
   - "N-O-T" program: offered in schools and community settings. Consists of 10 voluntary sessions where participants can talk about issues they are facing. Offers life management skills as well as ways to deal with stress, relationships, smoking, etc.

-Modeled under the BEINGS model and Health Belief Model

-Goal was to alter an individual’s perception of susceptibility and severity to racquetball eye injuries, ultimately increasing the use of protective eyewear

-Used pre and post survey, found people didn't feel they were at risk nor did they know the severity of their "inactions."

-People responded positively to the informational leaflet!

-physical in activity is almost as high as several well known coronary heart dz risk factors like cigarrete smoking, high bp, and high cholesterol

-Blacks greater chance of death from coronary heart dz than whites, asian least likely.

-It is recommended that everyone does 30 minutues of moderate physical activity a day (2010 goals are 30% for adults and 35% for adolescentts)

-Males exercise more than females

Title: Lyme Disease: It's Some Serious Poo

Main Points: (*=likely test questions??)
*Most common tick borne illness in US
  Caused by B. burgdorferi (spirochete)
  Only transmitted via tick bite!
*Doubled in incidence from 1991 to 2006
  Divided into 3 stages (primary, secondary, tertiary like syphilis)
  Erythema Migrans rash (bullseye rash) is characterisitic
  Treatment is oral Amoxicillin or IV Rocephin
  Can lead to serious chronic health problems if undiagnosed/undertreated
*Vaccine called LymeRix failed and was pulled from market in 2001 after 4 years of sales
*Chuck Norris once contracted Lyme Disease, but he was immediately cured when the infection   realized it could not survive in such an inhospitable environment and fled the bloodstream by re-   entering the tick.  The tick was then immediately transported into another dimension when tiny   "Charticles" (chuck particles) entered the tick's body and caused a cataclysmic rupture of the space-   time continuum since only one being can be made of true charticles in any given universe.  In his   new universe, the tick grew a beard, added a headband, and went on to an illustrious career of ass-   kicking UPN style.

- no specific tx, no immunizations yet
- can get an enlarged spleen/liver...  penicillins can give rashes
