Sannu (Greetings in Hausa) everyone,

               I made it to Niamey without any difficulty and as expected the heat was the first thing that I noted.  The country of Niger is such a beautiful contrast of red clay like soil and green shrubs from the plane.  It definitely is a place where the rainfall is scarce even though I believe we are in the rainy season now and it is humid

               I flew out of Niamey today to G and I have a lovely place to stay. (Names of places and people will not be mentioned for security reasons so please note accordingly.) We had a few glitches to iron out like the air conditioner not working well in my bedroom but I found out that it can only cool to about 86 degrees which is better than the rest of the house which is between 92-96 degrees, no working outlets in the living room-kitchen area, and a refrigerator that was not working well.  However everything appears to be working much better today and the air seems much cooler with the fans so I am blessed. 

               We had a teaching conference this morning and then I was given a locker and an operating room.  Somehow I started out with the visiting ENT surgeon, TS and started to help her with a case by removing a part of the mandible!  I had learned how to remove teeth in Ecuador and decided to take it a step further and remove a mandible with teeth using one of my favorite forceps!  Apparently the patient had in retrospect a mandible that probably was fractured in 2 places and had died causing a huge infection with the bulk of the mandible acting as a sequestrum-infected piece of dead bone.  She managed to get the rest of the mandible removed under Ketamine since we could not intubate the patient because of the severe trismus (lock jaw).  Then I ended up first assisting with a Cesarean section and doing 2 other Cesarean sections with a 2nd year resident YM- one with removal of a huge ovarian cyst and another with a tubal ligation.  We did have one general surgery case- a perforated appendix.  So that was my second day at here in GH and I had to leave for dinner so I missed a typhoid perforation in an 11 year old.  It is becoming very apparent to me that we often get 4-5 Cesarean sections a day and at least a few typhoid perforations.  So I expect to be a pro with CS by the time I leave!

               I apologize for not sending an email earlier because I have not had the time and I could not get on the internet.  Electricity goes off and one so many times during the day and night that I cannot count the episodes.  This makes it very hard on the computer and other electronic appliances.  Although this email is short and without pictures, I don’t have the time to compose anything since I have to make rounds tomorrow at 6:30 AM so I will work on getting something together this weekend, time permitting.  Thanks for standing with me.    Barb

Hi everyone,

I have had an enjoyable weekend off.  I just heard last night in church that Ebola has been reported in Nigeria which is just miles from our place in Niger.  Please keep us in your prayers.  So far none has been reported in Niger but we get a number of patients from Nigeria here in G.

Dorothy, thanks so much for sharing these with me.  Barb is such a

 wonderful person.  She could easily sit back and enjoy her life, but instead

 she chooses to go where most people would not, live in what we would

 consider wretched conditions and take care of these people without the 

luxuries we have here.  My oncologist goes to Africa one month every

 summer to help.  He told me they are lucky to be able to get an 

x-ray machine, let alone anything else.  He is a wonderful man too.  

God Bless all that give of themselves to others in whatever way or 

manner.

I got this email from Barb this morning.  I'll add this message to her thread, but I wanted to ask urgently for your prayers.

I received an email this morning.

"

This is to confirm that a patient with suspected Ebola virus came to G earlier today (Wednesday).  A young man who became ill while working in Lagos and returned to Niger (their home is  SK just east of us) today by his brother.  Since being here, he has shown no symptoms. 

 

He is isolated in the room at the east end of the old OB unit for observation.  His brother is not ill, but as they traveled together and were in close contact, we will also monitor his temperature"

This is a suspected case and not confirmed.  So far he is not manifesting symptoms of a hemorrhagic fever but would appreciate your prayers for the staff and our patients.  

Many of our patients are extremely sick with multiple diseases and malnutrition.  Surgeries that would normally heal well in the states do not do well here because of the poor state of the patient.  Wounds fall apart, abdomens open up, and the bowels break down easily.  As a result, please continue to pray that the Ebola virus would not come to G and that this patient does not have Ebola since the spread of this virus will wipe out a lot of the healthy people that are malnourished and anemic in the area let alone our patients who are in worse shape.  Secondly, many of these patients do not know the Lord and this is a far worse consequence than the former.  

Thanks for your prayers as always.  I've got to run to the hospital but I wanted to get this out so that your supplication to the Lord will uplift those here, especially those caring for this particular patient.   Barb

 All I can say is wow.  I thank God for all the people that have callings to be missionaries.  I pray that he gives them the strength to carry on against tremendous odds against them.  I pray that ebola is wiped off the face of the earth.  And I pray that Barb gets safely home again and in good health.

Want to get this email out before the internet goes down again.  We had a whomping wind and rain storm last night and I thought that my tin roof was going to blow off.  Instead it just sounded like a train was going through my bedroom!

Served dinner to the Surgical Department- ie surgeons and residents with families last night.  It was a fun but challenging time.  Cooking for a crowd anywhere can be a challenge but especially here in G as the electricity goes off and on amidst other issues like oven with numbers from 1-6 or 8 and that means that 350 degrees could be 2 or 6 or 8 or absent if the oven is old!  Makes baking a cake more of an art especially when you have to make everything for scratch and you don't have a blender or beater.  =-)

Having trouble getting my attachment to attach!  Praise God.  It worked the second time.  Well here goes.  Miss you all.   Barb
